Portsmouth Regional Hospital is seeking a compassionate and detail-oriented Registered Nurse to join our Coumadin Clinic on a PRN basis. In this role, you will provide specialized anticoagulation management to patients receiving warfarin and other anticoagulant therapies, supporting safe and effective treatment in a fast-paced, innovative clinical setting. Working within our Healthcare & Medical Services department, you will conduct focused clinical assessments, review lab results (INR and other coagulation studies), adjust medication dosages per established protocols and provider orders, and provide clear patient education on medication adherence, diet, safety, and symptom monitoring. You will collaborate closely with physicians, nurse practitioners, pharmacists, and other members of the care team to optimize patient outcomes and reduce complications such as bleeding and thromboembolic events. The Registered Nurse in the Coumadin Clinic will perform telephone and in-person patient follow-up, triage anticoagulation-related concerns, document all interventions accurately in the electronic health record, and help coordinate care transitions between inpatient and outpatient settings. You will contribute to quality improvement initiatives, follow evidence-based guidelines and hospital policies, and participate in ongoing education to maintain current knowledge in anticoagulation therapy.
